I have had the same thing happen. For some reason the diagnosis boxes above have to be checked off before writing the order. I think they are check off automatically with the first order but after that have to manually selected.
Also note that custom orders have to be "added" before they can be printed, whereas previously you could print them prior to clicking the "Add" button.
