Hocky, I think this is an excellent idea. I've been meaning to post such an idea myself, but you beat me to it.
We also have a screening questionaire that we would like to find a way to integrate into the system w/o having to have to scan it. Its a mood disorder questionaire. We'd like to be able to somehow have the patient fill it out electronically and then save/import it into the chart.
