Luckily, the real date is at the bottom, but I do find it very confusing. I would like your take on the fact that it seems somewhat of a medicolegal issue as well where you can just make up a note and date it back.
Maybe that should only be addendums where it states addendum and makes it very clear this is an ADDENDUM to a past note.
