I have several forms my wife created at the patient end of the portal, a demographic form, a prescription request form, an appointment request form, and the (thanks to the new law this year) annual wellness medicare screening assessment form. The patient fills the forms out and then sends through the Updox portal and then my nurse copies and pastes or routes into the AC message area. So the patient does all the work, and each form makes a nice readable message or chart import entry depending on how it is routed by my nurse--jimmie
