you cannot edit an encounter once it is signed. You can create an addendum, but cannot delete the note. It's part of the legal security required of an EMR. In the event of a lawsuit, the lawyers don't want us deleting important office encounters. So we are locked out from removing office notes. Every note would get counted toward MU.
