I use Dragon 11.5 Premium --I think it is called-- instead of medical. I use Microsoft's Remote Desktop to get to my office computer and do most of my dictation at home early in the morning.
There are some real quirks in using Dragon in the remote desktop windows, but it works pretty well for the most part. For example, you can't use the correction features unless you use a Dragon dictation box and then transfer to the EMR window. It is inconsistent about capitalizing the first letter of a line when you dictate directly into the EMR window, and often it puts an extra space in front of the line.
On the other hand is very accurate and works reasonably well. It doesn't seem to work very well in noisy environments even with a Philips Speech Mike which I have found to be the most effective microphone, which is one reason why I like the quiet early morning hours for dictation--besides the fact that my brain is more ready for that task early in the morning.
I suspect that the expensive medical version would not work any better than the premium version the way I use it, because I'm not using AC on the same machine with Dragon when I dictate remotely.
