I have been using Dragon 8 (medical suite) for the past 2 +years and I find it to be very efficient. I dictate all my notes, often between patient visits. I use Philips speechmike microphone. It hooks into a USB port and is handheld so you can quickly pick it up and put it down without trying to put it around your head/ears. It laso has mouse controls which comes in handy. It's a bit pricey (around 150) but I think is worth it. Good luck.
