Having been at the table at ACUC 2011 that discussed Orders as a subject for improvements and enhancements, my understanding was that there is much to be fixed/improved, including reconciliation.
I have heard several work-arounds, but don't know any practice that does use reconciliation. Perhaps there is someone out there who does who can weigh in.
