You may be right. Oncloser inspection there is a copywrite sticker on the bottom of the page on several pages. However, borrowing from them is probably legitimate, espeically as I have in the past purchased a copy.
I had not looked at the Bright futures website in some time, but it had an interesting statement that said:
"To incorporate forms into an Electronic Medical Record System
or to make multiple copies of specific items,
please purchase a complete Tool and Resource Kit online."
However they also have all of the relevent forms online. Since primarily you are recereating the development, questionaire and guidance sections, all of that is available online.
All it takes is cut and paste to create a template.
I may have to look at updating my templates, something I have not done in a long time.
