It seems to be a little confusing where to file documents for patient seen in the hospital on AC. I mean H&P Consult notes I dictate, or other physican who have seen the patients at the same hospitalization. There is no enough heading for these document to be imported to from the scanner. Is it possible to make it as an addendum and to code it. if yes how to do it. I would appreciate any input.
