I have been using the Excel formatted flow sheets now for a few months, and really like them. Unforturnately, I don't know how to attach an Excel file to this blog. Therefore, use as an example my Coumadin Flow Sheet. Each column has a heading like date, INR, etc. Every time I add a new lab report, I will open the flow sheet, add the info., and then save it.
Example:
COUMADIN FLOW SHEET Patient Name Birth Date
Goal 2-3 INR
DATE INR PROTIME DOSE CHANGE DOSE NEXT INR
We do the same for Diabetes, etc.
In the Tips and Tricks grouping in the Blogs, is a posting by Bert on ”How to make a flowsheet or facesheet for AC” which is excellent, and where I got the ideas on how to make mine.
