Yes! I've lost several notes. Usually its because I switch over to the demographics or summary page and then close it rather than switching back to the 'most recent encounter' tab. Even though I've made the same mistake several times, when I get to multitasking, it just happens. Even a pop up that says "are you sure you want to delete?" would be helpful.
