Maybe this is only an issue when one prints out the progress note, but it will frequently run to 2 pages & when this happens ther are two annoying results 1) the first page doesn't have my name at the bottom (and it doesn't even say page 1 of 2) so I have to use a rubber stamp and sign page one 2) page 2 may have only a single line or even one word. Is there no way to "make it fit" (that was a handy function in Wordperfect) ?

I think putting the signature/footer on each page is a good idea. I do think that we should all use the suggest improvement button to send to AC.

Definitely. There should also be -- maybe you are saying this -- but a "time" with the date stamp and the name of the doctor on the visits page. Maybe say something like:
Patient seen and examined on 06/09/08 at 9:25AM.

Agree, my problem is that I use the letter writer function to print the pre-op H&P. I send a letter to my patient. Then I delete the Dear Frank greeting and substitute a template, " This is the pre operative History and Physical Examination for this patient. This is a representation of the medical record in our office for the exam on this date." Then I type in all caps" PRE OP FOR JUNE 9, 2008 WITH DR. SMITH AT DOWNEY OUTPATIENT SURGERY CENTER" THIS PATIENT IS CLEARED FOR SURGERY"
Then I have to add a line at the beginning and the end with the two forms of ID, usually the patient name and date of birth. I will add the local hospital medical record number if I know it.
The problems I have are: 1- Same as Quann, I have to ID the second page or frequently only the signature is on the second page. I type the ID at the beginning and again at the very end. This works unless it runs to three pages, then the middle page has no identification at all. I hand write it on the top of the page along with the page number. Doesn't look as professional as I would like, but HEY, it is done! And before the patient leaves the office. 2- because I don't know if the report will be 1,2 or 3 pages I have had little luck trying to print the top page, (which is last out of the printer)on our letterhead which just looks nicer than the instant letter head AC generates. When I get done writing all the stuff on the pages I guess it doesn't matter what letter head I used.
