OK, now jokes aside, I am either going crazy and seeing things, or this is simply AC getting back at me. But this truly is the strangest thing I have ever seen happen. I had to upload it so you can see it for yourself. OK, so I had this 18 year old male patient up and was looking at his medications.
You will notice on the top row you see his address and telephone number and weight: 349 pounds. You see the height and the last date he was seen. Now below that on the next row you will see 44.99 kg on 6/25/2025. That is not his weight. That is another patient's weight. When I pulled up the girl's eRx, it had her address shown, but his phone number. I wouldn't have noticed except I called the phone number on the girl's script, but it was his phone number. When I closed her eRx and reopened it, it was correct. These were two different prescription windows and they weren't overlapping. Now, I am sure someone will tell me how it could have happened. The weights are so wrong, I couldn't have made a mistake. But think if his weight were 150 lbs. I would have overdosed by double.

Very interesting, I have had other issues where some information seems to jump from charts. I recently had an issue where while opening one chart I hit the next chart and tried to open it as well. That caused some kind of glitch where the second one opened and the first one went to the deleted box.

I have never see this type of error, glad you got a screenshot.

on 12.3 I have seen medication names change suddenly on current medication page. usually switching from some allcaps stand and the usual first letter caps. Weird.
