Besides Wendell, Raj and me are there any other pediatricians that frequent the board or use AC at all? Like a fourth one?

Hi John,
Must be pretty busy there. If you have time, feel free to comment more. The board is slanted toward Family Practice, which makes sense, so there are issues with AC that aren't obvious to others. Like Kg on the vital signs.

Here's an AC issue that effects pediatrics. Why can't we get the automatic text message sent out when e-prescriptions are sent? AC turns this off for patients under 18 because HIPAA they say. Every specialist I take my kids to.. we get a text msg when an eRx was sent. Not really a HIPAA issue.
This automatic text msg would fix a lot of problems of pharmacies telling patient "we don't have that medicine for you" and the patient calling us asking why we didn't send the eRx. Ninety five percent of the time, when I look in the chart, the eRx was sent. I believe pharmacies are not filling meds usually because it's an OTC med and not telling the patient anything other than "we don't have it." Patients take that to mean, the doctor didn't send it. We get this kind of call about 5 times a day.
(The doctor sends OTC meds because Medicaid covers it if there is an prescription..)

OMG, I hope there are no pharmacists on here. They drive me crazy, and I am sure I drive them crazy as well. The problem is they don't say "We don't have it," the say, "THEY didn't sent it." AARRGGHH.
And I agree with your point completely. I think there are so many rules and regulations that actually slow down and hurt healthcare.

So what do you/your staff do? Do you resend it? We used to "re-transmit " from the NewCrop page. But recently in the past few weeks, the "re-transmit" says failed due to duplicate. I don't know what's best.. print it out and fax it? Or tell patient, look it was sent and we can't resend? Almost like paper prescriptions would be better for OTCs. (Better for us staff who have to field these calls & resend, etc.)
The pharmacies in Houston are so busy. I understand patients would rather call us and get it done again. Trying to talk to a pharmacist means long old time. But it puts more administrative burden on us.

It is too much trouble to call and stay on hold to talk to the pharmacist or tech. It is much simpler to resend the meds, because the medication will be available in the "write Rx" window in the unfinished chart or "refill meds from the dropdown window when right click on patients name in the patient lookup window. I have not had any "repeat offender pharmacists" yet who frequently tell patient's parents that "they did not send it".

There are two kinds of pharmacies. I will use pharmacies and not pharmacists. There is the pharmacy that will see Doxycycline hyclate turned down by MaineCare due to cost and reject it for a PA, and the pharmacy who will automatically change it to monohydrate so it goes through. There is the pharmacy who calls the office and says, if you change this to monohydrate it will go through. Or the pharmacist who tells you that 2.5% HC comes in a 22 gram tube and not 15 gram.
And, why the hell can't you write the word Brand in the message section of NewCrop. You have to disguise it and write BND or Brande.
Of course, there is now a national shortage of amoxicillin. Amoxicillin! The go to drug for pediatricians. Cefdinir and Cefpodoxime are a toss up as to which a pharmacy has.

We have a lot of these repeat offenders that tell parents they don’t have the rx. But we cover such a large geographical area we deal with so many pharmacies and we can’t keep track of them. And parents only want pharmacies near them. I’ve tried to send them to certain “good” pharmacies but they don’t want that bc it’s not in the right location.
Yes the shortage of amoxicillin and all the pediatric drugs makes this more burdensome.

I end up sending two scripts. One regular script for Cefdinir and in the notes section, I write: If you don't have it, then please prescribe Cefpodoxime 100 mg/5mLs, 4 mLs po bid for 10 days times no refills.
