We have always had new patients complete a questionnaire prior to their visit. Usually we would mail them a package and encourage them to complete the form prior to their visit. We then scan the form into their chart, and use it to enter pertinent demographics (at the front desk) and clinicals (by the physicians) into AC.
There has always been some value to sending and receiving those forms electronically, but with so much more telemed now, there is greater urgency to find a good solution.

The form is 3 pages, and is currently a Word document. Of course it can easily be converted to a pdf or even a fillable pdf.

The question is how to most efficiently get them to the patient, have them fill it out, and then return it.
Of course if the patient has printing and scanning, they can print it and scan in the completed form and send it. Even photographing the completed form and emailing is possible, but not ideal.
Ideally, we would like patients to do this without a download or (at least without printing and scanning) and if possible, to do it on their phone.
HIPAA compliance is important as well.

We have Updox and I created a thread to discuss this (https://acusersforum.com/ub/ubbthreads.php/topics/75657/updox-forms-how-to-use-them#Post75657) but I am opening the discussion up to non-Updox solutions as well. Of course I would be happy to hear details of how people do this with Updox.

This solution has been absolutely fantastic for us. We have converted all of our forms to electronic and the forms can be emailed, sent via text or hard copies can be printed. The forms also allow for electronic signature from patients.

The price is right too!

I have recently created some Online Forms for patients to Fill before their visit to our office - as we are using Telehealth. Here is the link my office send to the patient via text message and they get to Updox Portal to fill in the forms. You all are very welcome to see it how it looks like from patient's view point. Very quick to fill in and submit.
