How do I attach patient log-books with Patient-Generated Health Data entry in Alerts and Directives?
I followed the following instructions but do not known who to attach / link document imported in Imported Items with this entry. Kindly help.
How to do this in Amazing Charts
Denominator
Patients who have an encounter with the clinician during the reporting period will be counted in the denominator.
Numerator
Patients with information added to the Patient Generated Health Data screen will be added to the numerator. In the Alerts and Directives screen, you can also keep track of patient-generated health data.
1.Select Patient-Generated Health Data and click Add New.
2.Fill in the Level, Name, URL and Reference fields. (If there is not a URL, type in none - the fiel is required.
3.Click Save. The Patient Generated Health Data information will display alongside Alerts and Advanced Directives in the Alerts & Directives tab of the patient's chart.
