Not sure if this helps. I use Updox to create a form. Patients browse to my Updox webpage
https://myupdox.com/portal/<yourNameHere>/html/publicform-<someNumber>.html. They don't need an account or password (e.g. new patients). They fill out one demographics form (insurance, address, etc) and one medical form (list of meds, past surgeries, etc). When they click send it arrives as a fax in Updox in my general mailbox, but is it a fax from which all entries can be copied and pasted by my staff into a new patient chart. Granted, each field needs to be separately copied and pasted. Alternately you could just import the form into the imported items. In either case, it does not auto-populate the fields in AmazingCharts. The latter would be a welcome development.
If you could get your established patients to do it before each visit, I suppose you could create a follow up visit form, have them enter data for each Progress Note text box (e.g. History, ROS), and just cut and paste into your note.
