For our health information exchange, we need to generate a list of active patients. How do I do that to include addresses? When I run the report based on schedule (seen in last 12 months, for example) I get all the demographic data I need except for the address fields...

Hi Jon, You can generate a list of active patients with their address using the following query criteria:
Operator Category Fields Value
---------------------------------------------------------------
= Demographics Inactive FALSE
<> Demographics City
<> Demographics Street Address
<> Demographics State
<> Demographics Zip Code 00000Let me know if this helps!
