I have Acrobat Reader DC on my computer and I am trying to add a patients Name and DOB to a PDF. The document is already in Imported Items. When I click on Comment everything is grayed out.

I don't believe you can add comments to a PDF document once it's been imported

If you want to just add a comment without going into the PDF, you can do that.
Right Click on the PDF in the I.I., Click Edit Item Description, type away in the text box below!

Welcome to the AC user board. Hopefully Boondoc's answer is helpful to you. I thought that Marty's answer was correct, but I no longer use Acrobat (I use Foxit for imported items) so I can't verify that Chris is correct.
I guess I wonder, though, why you would want to add the patient's name and DOB to a document that is already in imported items? It is already in their chart... I am not trying to make trouble, just wondering how this would help you.

What Chris was talking about was adding comments not directly to the PDF document but to Amazing Charts (same area where you file where the PDF goes, put in dates etc).
