We had a new PA start at the office and for some reason some of her charts are being signed multiple times before forwarding to the Dr to sign off.
The signatures seem to be duplicates because they are at the very end of the note and have the same exact time. And just a couple blank spaces between each.

The only thing I could think of was the User Preference that timestamps every edit made, however, being that it is at the end of the note and same time it doesn't seem likely. It couldn't be that she is forwarding/reopening the chart to herself that is causing the duplicate timestamps, right?
Any other ideas?

First thing I would try is to completely exit out of AC and restart it.
