I have noticed that the second and subsequent page shows the patient's name but no date of visit. This is a problem when we need to send records to L&I and insurance companies. We make a hard copy and then put down ("DOS: MM/DD/YY") by hand which looks unprofessional. Also it is difficult to FAX notes because the fonts are too small. After FAXing, we have been asked to send hard copies as entities examining the records have difficulty reading the notes.
