I think that once it is signed off it is done...only an addendum could help here, otherwise a smart lawyer could accuse us of altering charts. Charting is such fun..I know this makes me mad when there are still docs out there that write 4 unreadable lines on paper and go home on time.

Thanks Todd, I'd hoped we could avoid having to write an addendum on an initial consult note, but alas, it was not meant to be... frown

Trista,
Todd is correct that once you have signed-off the note, it cannot be changed. An addendum is the way to go. Sometimes for an error like you describe, the best thing to do is to create an entirely new, corrected note (with the same date of service) and use that one, creating an addendum to the incorrect note stating that it should be ignored entirely.
Some of us would like to have AC create the ability to indicate corrections (for example, drawing a line through an error, just as with paper). Please send a message to AC if you agree that would be a useful feature.

Jon, ALL OF US "would like to have AC create the ability to indicate corrections (for example, drawing a line through an error, just as with paper)". I would like to have JUST ONE of my suggestions actually be implemented in a software upgrade. For example, I have asked numerous times that the send letter screen allow us to search the doctors names directly and narrow the search as the name is typed (as it does on the main screen, patient search)... not happening... Also, why is it when you search a doctor's name that begins with "L" on the send letter screen, it pulls up "send letter to patient"? Isn't there a box that you can check just above to generate a letter to the patient? Was it written that way simply to piss us off???

Ed, the second quote was from your first post, back in 2009.
All it takes is a little patience. wink

What I remembered is that you have made some good suggestions, and I knew that at least one was implemented.
Keep on suggesting...
