for meaningful use reporting I use the med rec button located on opening the medication box. when however i look to report for med rec for my medicare ACO patients i was hoping i would find text in the note or a date under the med list to show that i reconciled.
For my patients or transition from hospital to office i have documentation in my note. But for others who for example saw a specialist and then me i do not have the documentation in my note. anyone know if i can run a report that may show reconciliation? looks like i will have to add the verbage to my notes that i reconciled meds

Run a report that includes patients returning from specialist or hospital. Could use chief complaint. Fu after hospitalization or Ed or specialty consultation. Why won't the ACO use your meaningful use wizard report?
