I wish in future improvements - hopefully there will be:

-1. Ability to change the font size of the notes printed. My consultant says the prints are too small.

-2. Ability to add logo to the top of the note, to look more like say Hospital H and Ps.

-3. Ability to sign the note or prescription on tablet pc, with capture of signature.

-4. Ability to view the prior encounters on a wider screen as the current screen wastes the right 1/3 of the screen and of course when we print print, to print with bigger fonts.

-5. Have the option of adding more insurances in the demographics sheet as there option for only 2 insurances. This will be important when AC has its own PMS. My biller says many patients have 3 insurances.

-6. In the treatment plans- way of organizing the treatment plans into boxes- as the current treatment plans can look jumbled - the orders printed always go to the bottom, the medicines prescribed always go the top- though the medicine could be for a diagnosis way down in the list. Though everything is readable it would be nicer to have this in a methodical way per diagnosis list. So tabs/boxes for treatment plans would be the way to go. You take the cursor to that tab/box based on the diagnoses and document your treatment. (whatever diagnosis you listed will come on that tab).

How do other AC users feel?

Some of these have been mentioned in the past, hopefully Jon and his efficient staff will get this for us.

-7 Ability to check off that a import has been reviewed by a physician. As I try to go paperless, this is one of my biggest fears: that I will miss a lab report or xray report that gets imported and never forwarded to me. There needs to be a way to document that the report has been reviewed.

Right now, since everything is coming in via paper - labs, imaging reports, consultant notes, etc - none of it gets scanned unless I've initialed it.

That said, I look forward to the day that we can give and accept digital files - especially between offices. Then I will definitely need a way to check it off as reviewed.

I would like to be able to capture the ICD codes in the patient's problem list when I order test and put in an ICD code. Now I have to re enter it when signing the chart.

I would like to be able to retrieve and reprint orders.

I would like to be able to print an invoice when I sign a chart before exiting out of the chart.

I would like to correct an error after a chart is signed. For example we recently documented the murder of a former patient who had moved out of town but it went into the wrong chart. How to get it out?

Practioner/doctor read the document, and check off- Might work.

What we do presently is--

In a way similar to what Barbara said, if the document comes electronically, say by fax then,
I forward to my file clerk outside of EMR,
using an inbox for file clerk created in c folder of her computer,
with a shortcut to that inbox
- so she has an icon on her desktop- saying inbox,
then she imports the document I forwarded
and in COMMENTS section for import: types in Forwarded by Dr........ and Imported by .........her name. (she uses shortkeys: 20 bucks: http://www.shortkeys.com)
This way there is a comment that I forwarded, and of course if I forwarded I should have read and taken action on that document.

I guess with current system- the only way is to add a diagnosis in problem list first - of course it would have an ICD if it is ICDable (is there such a word??). Then one could print the order, all those diagnoses will flow to order sheet.

Retreive: With current version of AC: We have similar situations where patients lose their order sheet-we have to look it up in past encouters-- the orders given, then type up (well, we use templates) to print a new order again.

Re: printing invoice. I dont know, I am waiting for Jon to build an integrated PMS this winter. (Just cant wait, that is so exciting! to look forward to- this will cut back on double entries into EMR and our separate PMS).

Re: correcting entry, for legal reasons, I understand the only way to correct is to make another entry which says the previous entry was wrongly entered and to ignore. Then make a corrected entry. Once an entry is saved, cant get it out, it is one way street, this I believe is true for all EMRs.

Same idea. We're going to be getting Lab results via the web to attach to the patients charts with the AC interface soon. Not only do we need a way to show that the doctor has reviewed it, but to follow it, such as did MA or Nurse make a phonecall, mail a letter that all is normal, make appointment based on F/U to lab and so on.
Also, very Important. Make this part of reminder system or something. There should be a glaring reminder on the Doc's computer that reminds them that there are fresh imports or labs to review. What we do at the front desk needs to be put infront of our Doc in a way they will remember to look at it later....In Box or Reminder type thing, flashing red????

I receive all my fax online via email as TIF (better than pdf i think). My MA imports it directly into the patient's chart and forwards it to me for review. Currently i use windows piture and fax viewer to write a note on the actual import and save it. If I am using my tablet pc, I can literally write on the report. If I am using my desktop, then I just type over it. It is a good method, but I am still afraid of missing a report. I would love to have a way to place a checkmark next to the import to document that it has been reviewed.

Ah I just remembered another wish--

When you do your assessment- like hypertension or gangrene, I wish it would flow to Past medical history in addition to problem list. What I am asking for may have some drawbacks though, as where will you insert this new diagnosis (or how will AC know where to insert this new info) when our format in past history is--

Immunizations................................
Advance directives...........................
Tests..........................................

I think what Carol suggests is an excellent idea. -- batch printing all messages.

The other suggestion mentioned by another user-- search all the labs ordered for a particular patient is an excellent suggestion. Patients lose the order, instead of starting all over again, search the order already given, reprint and give to patient or fax.

There are several improvements that can be made at the superbill areas. For example that the 1500 can generate from the information on the demographic in terms of the amount paid by patient. Item 29 of 1500. Also that the information regarding place of service is enter to the 1500. At this time is only placing office (11).

At our office all our providers use tablet pcs. We sign imported items on our computers just like we used to do in the old paper days. When we close the imported item it is saved with our signature. The software interface for our imported items is Microsoft Office Document Imaging which is a part of Office for Small Business 2003

Why batch print messages? Why print anything? I must be missing something. We don't print anything that doesn't walk out of our office in a patient's hands (prescriptions, orders, back to school notes, etc.). We have 5000 active patients and have so far this year used less than 2 cases of paper. Think of the trees we've saved and the money I saved not paying employees to handle paper.

Jon and team:

Thank you much for large font size note on v3.62. You are a team of geniuses.

To other AC users: download v3.62beta. Go to consultant/letter writing section, choose, the large font section and there you see it, the full picture with big fonts in your eyes. My surgeon consultant will be happy.

I second the motion. There is one little bug, however. The practice header now is aligned on the right margin as opposed to centered on the page. This must be fixed asap.

A nice feature would be the ability to attach the progress note to a particular imported item. For eg., I saw this pt today, went over the MRI and a lab. It would be nice to attach the progress note with a paper clip icon (similar to email attachment icon in Internet explorer) to the lab. So if you want to send that batch of papers to a consultant, you click one and get all to fax to consultant.

Gcarcamo: Jon mentioned something about double window envelopes #9; do you think the letter modification is towards that step- left align the letterhead and TO address??

I have no idea what a #9 envelop is, but I guess you might be right. However, if that is the case, this should be an option and not the default. My letters look very "amateurish" at this point and, for now, I am having my office staff write the letters instead. Lord knows if I received a letter that looked like my letters do now I would not be very impressed at all. I am not pleased at all with this "enhancement." I do appreciate you pointing that out, however.

One more thing, I appreciate the fact that we now have the choice to print the notes in a larger font, but I prefer the smaller font. There should be a way to make the option of using the user's choice of font/type of note/practice letterhead vs not/etc permanent so one does not have to scroll to the correct option each time when printing a note. It is very annoying.

I realize I may sound a bit annoyed, which I am, but I do not want people to get the idea that I am displeased with the program itself. I highly recommend it. These issues are only cosmetic and, I hope, easily corrected. This is the purpose of being a Beta user, to find those bugs/issues that may be easily corrected before releasing the product to the other customers. I commend Jon on his program and wholeheartedly endorse it.

I agree with you about the left alignment of letter head. It looks too amateurish now on my Diabetic supplies and other DME supplies scripts.

I wish it was like old days with the letterhead in center.

Hopefully Jon will get us the central letterhead to make it look more professional than that window dressing (window #9 envelope)

I AGREE ABOUT THE LEFT ALIGNMENT. WE SHOULD BE ABLE TO CHANGE THE FONT AND ALIGNMENT OF THE LETTER.
ALSO IF YOUR LOGIN IS DR SMITH, YOUR SIGNATURE AND NAME ON BILLING SHOWS UP AS DR SMITH, JOHN. IF YOU CHANGE IT TO YOUR NAME FIRST IE JOHN SMITH, M.D. THEN YOUR LOGIN IS JOHN AND UNABLE TO DESIGNATE MD FROM STAFF IF YOU HAVE SAME NAME...

We stopped printing messages a year ago and haven't missed not having the hard copies.

However we still print progress notes.

As an enhancement--

Would be nice to have the ability to open multiple charts at same time-

Would be nice to have referral management as mentioned in another thread--

Would be nice to have the ability to send email from within chart (when the chart is still open) to another staff re: say an appt rather than jumping to the main screen, search the patient and then send the email---

Would be nice to have the ability to have a paperclip icon for that progress note- clip the relevant labs, xrays- that were discussed- as mentioned in another area----

Thank you Jon and team for the constant improvements you are making in AC, still keeping it simple, most importantly downloadable from internet, so we don't have to wait for 3 mths to get an appointment for upgrade at our office servers (unlike other EMR programs where every user seems to be at different version).

Fantastic.
