A new patient just told me of this great app for Chrome and gmail called Hello Sign. It lets people fill out and sign forms online. As far as I can tell, you need a gmail and /or google drive account (which are of course free). You might need to use the Chrome browser, but I'm not sure on that. All you have to do is upload the document to your google drive and open it with Hello Sign.
They claim it is legally binding, but I don't have any proof of it.

After looking into it some more, it may not be a good thing to trust yet. So you upload a copy of your signature to Hello Sign, then they get hacked and now all sorts of things have your signature on it...including the loan for $1,000,000 from some strange bank in Columbia.
