Yes, a Care Plan can be documented in the Plan section of AC. Medicare doesn't specify a specific format and content for a care plan. But generally expect that a care plan should address a care management plan for the elements typically reviewed in an Annual Wellness assessment and exam. Insurers also accept template driven Care Plans populated with patient specific information and care recommendations. Evidence Based Medicine guideline references for your care recommendations should also be included. The care plan is a collaborative process between the patient/caregiver and the provider and includes your care management recommendations as well as a patient centered self-management plan (essentially what the patient agrees to work on for their own health benefit outside of the office visit with you). Manually creating a care plan for each patient would be require much effort and be prone to omissions in my view. My practice uses MyPHTS web based application computerized history tool to facilitate Annual Wellness assessment documentation and creation of patient centered care plans which we then incorporate into our Amazing Charts notes. I am part of a team of primary care providers who created this tool. I use it and AC for my practice. We are also engaging our Medicare patients in the new Care Coordination Management services code 99490. MyPHTS and AC have been vital to this.
