Is anyone using (Does AC have a section?) AC for Chronic Care Management?

We are looking at starting this in the next few weeks and I know the key will be proper documentation to avoid RAC audits.

In particular a "Care Plan" and documenting time (non face to face). Can a MA directed by me be sued to document the plan and time?

1) Yes and No - AC alone is not sufficient in my view for documenting Chronic Care Management because of the "electronic Care Plan" requirement. We use MyPHTS (www.MyPHTS.com for $49.99 per provider per month; 1 month free trial) automated web based tool for annual wellness documentation, patient centric care plans, chronic care coordination, and patient education and self-directed management tool. We leverage MyPHTS with AC for electronic care plan documentation.

2) Yes - proper documentation is key; MyPHTS facilitates compliant documentation

3) But CCM is not just "documentation"; CCM is a process involving some level of practice change management and adaptation; it's not as complicated as it would seem once you get your processes in place;but you do need to be invested in adapting some level of change; MyPHTS team assists you with this process by learning about your practice situation and sharing insight and protocols for incorporating CCM into your existing practice processes.

4) Can MA directed by me be used to document plan and time:
Yes - My office MAs use MyPHTS to assist in obtaining automated Annual Wellness and Care Plan assessments from patients. We incorporate the assessments into our AC office notes elaborating where necessary.
Yes - But only certified/registered MAs time can be counted towards the 20 minute non face to face CCM time documented. I am encouraging my office MAs to pursue formal certification/registration.

Anybody have any thoughts about chronic care management (medicare code 99490, $42 a month) and amazing charts? I notice Amazing Charts has a new partner in this regard, caremerge.
I'm a little leary about starting Chronic care management after reading about all the bounty hunters / audits going after meaningful use money.

Personally, and professionally, I wouldn't touch Chronic Care Management with a pole-vault pole.
1. You get $42/mo -- IF you jump through endless hoops. And figure out how to collect$8/month from the patients, who haven't the foggiest idea why they should be paying for something every month when they don't even see the doctor
2. You apparently can't do it with the tools you already have -- here we are presented with one of several "add-ons" to AC (for "only" 49.99/mo per provider) -- yet another money and time suck on dwindling reimbursements.
3. Opens wide open to audits -- did you REALLY dot every single i? Spend thousands of dollars and countless hours, going through an audit -- when you maybe collected a few hundred dollars a month.

No, I think I will just continue to insist that people come in to be seen. Sometimes you can be surprised by what you can see (and feel and hear) when you actually examine people.
