I am trying to set up patient portal. I do have Updox that I used last year. When I open Updox I don't have the "AC user mapping" option under admin. Do I have to buy the $50 per month Updox to use patient portal or am I missing something?
One if by gland, two if by A1C...

What version of AC?
Also, from experience, if you had Updox, switch to "free" Updox with v7.x or later, there are some things that have to be done to get the systems to talk to each other.

Question answered by Mark at AC.
...you do not have to have the full version of Updox to use the direct messaging or portal. With V7 and above, you can simply activate the portal through the admin options and star using the features (Click here for a walk through on setting it up and using: Patient Portal )...
