So let me try to explain and see if anyone else has seen this problem. When a patient is seen and the bill is printed the diagnosis code on the bill may or not be the same as the diagnosis on the visit. For some reason, the printed bill may have old diagnosis show up that are not related to the current visit. I have checked this and it is no a user entry error. I have sent this to AC as a critical bug but since we changed over to 7.1.3 a couple of weeks ago I have found several small issues. I have not heard any response from A/C. I sent these either as a bug report or improvement suggestion. I have even asked for A/C tech to call but have not received a call. There are things I like about the Upgrade but other issues that were correct back in version 6.6 that are now problems. We have used A/C for years.

Yes. and it has continued to occur through multiple versions. I leave it to my biller to scrub the claim

Question: Are the bills being created before the chart is signed, or are you generating a bill upon signing off and the biller is just reviewing that bill? I ask because billed that are created outside and encounter will populate ICD codes from the Problem List and not from the chart (nice they are created outside of an encounter). Also, removing the hashtag (#.. the number or pound sign as I remember it being called) could also cause the ICD codes not to go over to the superbill properly.

Ok, I have been trying to post this for the past several days but keep getting kicked out. I have noticed that the diagnosis codes populate while giving an immunization. A patient may come in for a flu shot and while adding this by using the decision support window the injection code and immunization populate onto the bill automatically. The problem is it pulls up any diagnosis in the patient's history and does not use the one put in during the visit. I make a chart for the patient receiving the flu vaccine so it is saved as part of the EHR. If they are there only for the flu shot then the diagnosis is v04.81. It does not matter if this is the only diagnosis code for the visit or whether I put in the code before or after the pt receives the injection the diagnosis code does not populate correctly on the bill. We have had to change each one as they are given.

Thank you for the clarification Chris. I was able to replicate the issue on 7.1.3. It looks like the diagnosis codes pull from the previous visit notes and not the current visit notes. I'll do some testing on version 8 to see if the issue has been corrected. If not, I will see what I can do to have this addressed.

Is this only happening when you are printing the bills out? The reason I am asking Chris is that we are currently not using the superbill function in AC but I was going to start having my provider do that next month.

Yes, we started printing super bills out a few months ago to transition from an old paper form. The 6.? version we were using did not change or affected the codes this new 7 version does. We are transitioning to Kareo and will monitor this as we start sending in electronic claims. I noticed this is with any immunization documented using the "Decision Support" tab. This version is great in that it puts in the code for the immunization and the injection code (Medicare patients are wrong, doesn't use the G codes) but the VIS sheet information is not auto populated and does not even remember from the last time you gave the same immunization. Example: you have to put in the VIS version Date every time you give a flu shot.
