1) We do a lot of clinical trials in our practice. We need to print the entire chart for patients that choose to participate. I went to print a complete chart and it would have been "520" pages long!! Almost 500 of those pages is the social history duplicated over and over. I seem to have numerous patients with this issue. How can I have the "social history" repeats removed from all but 1 visit (encounter) or not print at all?

This has been a problem for some time and we are unable to locate a fix.

2) How can I print encounters for on a certain time frame (exp. 2 years) without having to either print "everything" or having to print one encounter at a time?

We have had the issue;
The staff has to be diligent about removing the duplicate;

I am using 6.3.3 and it is a known issue.
If you select the smoking status first and then enter other information into SH, it will duplicate the text every time you use the chart.
If you enter other text first and then the smoking history in the drop down box, it will not duplicate.

So, you have to train yourself to use the smoking box after your other SH.

We are currently using 6.6.2 but this is a problem that we have had for some time. Probably created in an older version. Biggest problem now is that once this has been attached to the encounter, it can NOT be removed. If someone requests a copy of medical records, I can not be printing out 520 pages of something that should only be about 20 or so. I want to know how to fix past encounters and have these duplicates removed. If 1 visit has the SH on there 50 times, so does the next visit and the next and the next. It just keeps repeating. You can'T even tell there is a problem when the patient comes in because the SH box is so small. HELP!!!

Once an encounter is saved, us regular folks have no way of changing it.
Only those with knowledge of and access to the database can change it.
You might appeal to AC tech support to see if they can help.

This highlights another difference between government/payer approved & certified software, and a system that gives the Physician/Provider ultimate control over their treatment record.

There is a small but growing revolt to allowing payers to unilaterally dictate to Physicians/Providers, and this is one of those pain points that will only get worse.

Not really a good idea to start changing encounters no matter how wrong it is by doing it through the SQL database. Plus, if you have 100 encounters, unless you write some type of script, that's a lot of PSHs to change.

I don't think I would ever print records. Burning them to a CD allows you to burn 10 pages or 500 pages in the same time without wasting paper.
