I was looking through the folder that details suggested changes to AC. I saw a reference to adding or modifying the main headers in the note, ie, CC, HPI, ROS,Med History, etc. It looked like fields can be added and the order can be changed. How is this done?

I have never seen a way to do what you are describing. Maybe I am misunderstanding, but the fields you reference cannot be changed.

Bert, I saw a modified note in the Improvement for AC section by a fellow named BruceL. Did he make his own graphic or are some of those fields modifiable?

That was a graphic example if you are talking about the one that shows text being put in fields that shrink to the size of the text, et. al.
