Has anyone else noticed a phenomena of Meaningful Use Documentation and BP values disappearing in forwarded charts (using V 6.5.6)?
A few months ago, I began to note the absence of previously documented blood pressure readings in the charts of returning patients.
Then I began to note this more consistently.
As an experiment today, I fully completed a test chart, especially noting that BPs, Medication Reconciliations, and Clinical Summaries were all correctly documented and appeared as green "OK"s in the Meaningful Use Wizard. I then forwarded the chart to my office manager in the same office.
When we opened the chart in her computer, the BP readings had disappeared, and the Meaningful Use Wizard now indicated in a red "Not Entered" that the BPs, Medication Reconciliation (though the box in Current Medication field remained checked) and Clinical Summaries had now vanished.
It appears that somehow, the act of forwarding the chart causes the MU Wizard to lose its previous documentation, and then document in red "NOT ENTERED" that Medication Reconciliation, Clinical Summaries and BPs were now absent from the chart (though the actual BP readings itself in addition to the MU documentation was gone)
Has anybody confronted a similar problem, or recognize a potential solution?

When we opened the chart in her computer Since this is a forwarded chart, when she opened the chart, did she pull it from her right panel inbox, or search for it and get it from the left panel "patient list" section? Getting in from the left might cause the BP's to be lost.

No that's not the problem, good thought though. But we are definitely opening the forwarded chart. It is unsigned of course but all of the data (besides the BP) is migrating. Interestingly the check boxes are still coming across as "checked" yet the meaningful use wizard is no longer recognizing them as being checked and instead marks the core/menu item as being "Not entered." Unlike the BP, which vanishes upon forwarding and leaves the meaningful use wizard legitimately without any data, the other items that are failing shouldn't be. Has anyone else come across this problem?

I also had some disappearing BPs. It seemed to occur if I entered the BP then forwarded the chart, if I had not moved down to the body of the PE box. I am not sure if this was 100% of the problem, but try this and see if it makes a difference.
I am using 6.3.3
I have not noted the other MU parameters disappearing.

I have noted that this occurs when forwarded chart is opened on a different. Date Then have to uncheck and recheck med recon Save meds Have not encountered bp issue Currently on 6.6.2

Hi All,
A few added thoughts:
I typically fill in the information fields in the order on the chart, so I typically add weight, Height, BP and pulse before filling in Physical Exam and Assessment.
I typically forward the chart immediately (within minutes) after I have filled in all the fields.
Of note, I was unaware of other parameters missing from the MU Wizard until I opened the Meaningful Use Wizard Box on my office manager's computer (where I had forwarded the chart). and found they had been unrecorded or lost - you may want to do the same to check your system (it may be happening unbeknownst to you)
A few other clues:
1. I've noted that if I carefully fill in the vitals fields, but then check the MU wizard, they are reported as "Not Entered" even if the data is right there in the vitals fields. - simply re-clicking on each vitals field (even though they are completed and already filled in) now allows the MU Wizard to recognize them as completed.
2. Same Phenomena with Med Rec - The MU Wizard reports it as not completed - I open the field and find that Med Rec was indeed checked and is highlighted in green, re-clicking on it then registers it on the wizard.
It almost seems as if these fields are "fragile" and easily dropped from the MU wizard - and even if completed on my screen, they are then lost upon forwarding.
We are going to call support, but I'm still curious if this phenomena has been noted by others or if anyone has any ideas.
