So, I saw a patient on Jan 15th. Presumably completed the visit, signed, closed, and billed the visit. I know that the visit was completed and closed because it was billed by EZClaim via interface transfer.

Now AC has no record of the visit. We have not had any obvious database problems, or errors from AC. So, where did my data go ? This is a real problem.

Did you check the deleted files list (hopefully did not clear the list ?). Maybe deleted by mistake by confirming delete without saving?

Just a thought on trying to retrieve the file.

I found the 'new' visit in deleted items.
So, I can recreate the visit, but I am more concerned with the fact that the original data is missing and there were no database errors noted in AC.

The visit was definitely completed or else my biller would not have been able to bill for the visit.

Even in the audit trail, the chart moved through completion.

I would leave that question to the database experts on the forum. Good luck and hopefully this is an isolated occurence.
