Currently we have scheduled hours 0730-1600. We've had a couple of occasions where I've wanted to add a patient onto my schedule at the end of the day. Does anyone know how to do this? We've resorted to simply double booking the last block, and putting the actual appointment time in the comments. This is clunky, but not as clunky as trying to actually extend the schedule for that day only.

One way to do this is to add a fake provider to the sked from 0600-2000 (or whatever that covers the time that you might ever see a patient outside of normal hours), and that makes the sked gray for you. So, you can still sked in those gray blocks, and the white blocks reflect your sked hours.

You can also change the schedule for 1 day. Go to administrator options to set provider schedules, but be sure to set the schedule period to just the date you want to alter, then set the hours.
Maybe this is what you thought was clunky, but it has worked well for me since it is only an occasional day that I want to change.

I just overbook the last slot with a time indicator:
