Is anyone working toward a Medical Home certification? We need help figuring out how to track outcomes for chronic illness.

Hi, Cheryl,
We are working on the massive paperwork for PCMH currently. I know this will be an issue that I come across, and would appreciate keeping in contact to share thoughts on the chronic illness outcomes tracking... and probably more!

We too are working through PCMH. Which element are you looking at, I can look at our policy that is defined for that element and see if we have something in place yet?
