I am sure that this is a simple task, but I have been unable to find an answer on the forums or website.

How can we export the contact information to an Excel spreadsheet? We want to capture the patient name and address including zip code.

It's simple once you figure it out.

Use the Reports tool.

To export all patient data, I'd so something like this:
0. open the Reports tool. the button to open it is located in the top left-hand side of the main window
1. select demographics, last name, <>, ldsjfaljf (or any other nonsensical string of characters); clock on the "add criteria to query" button
2. select demographics, street address line 1, starts with, (leave field empty), add criteria to query
3. repeat with street address line 2
4. repeat with zip code
5. run report
6. click on export button in the bottom right hand corner of the reports tool

Or you can go to Administrative Options (View + CTRL A) from main program. Select Import/Export. Under Export Data Out Of Amazing Charts, you can either select Patient Demographics, which will give you demographics on EACH PATIENT in a .csv file in the AC folder, which you could import into an Excel file or select Addresses of Households which will export an .xms file of all HOUSEHOLDS directly into an Excel sheet. This is possibly your best choice since there may be four patients at the same address.

Your format that you suggested is right on for running a report from the report section, but can I make a suggestion to make it a little easier?

First off, you don't have to worry about running a query line for "Last Name". This will be in the report anyway. Second, I would suggest changing the value field to "%". This is called a "wildcard" and will automatically search for any value entered into that field, even if it is blank (as some address line 2 fields are).

Overall though, Bert's suggestion is probably the best for getting unique household addresses. I would suggest using Mario's method if you further want to narrow down that list (by say adding a criteria searching for all your diabetic patients).

@suggestion #1: haha, I run reports a few times every week and I never add criteria for name and last name. Yet, I included "add criteria for last/first name" in my steps; funny how the mind works.

I forgot to include "check the 'show all query results' button at the bottom left hand side of the reports windows."

Still. For exporting demographics, I like Bert's method more. It's faster and cleaner.
