I forget....what is menu 4? You have to go into the admin area to activate the decision support that will prompt you to address things like colonoscopies, immunizations, mammograms etc. in the plan area.

Send reminders to patients per patient preference for preventive/follow-up care.
HOW TO DO THIS IN AC: From the main Amazing Charts window select Reports, then select the highlighted Meaningful Use Searches menu. Then choose whether to search on patients <=5yo or >=65yo. You will be given an option to filter by contact preference. Selecting All will include patients with and without a contact preference. Then you will need to add at least one more criteria from Demographics, Diagnoses, Medications, or Laboratory Data. Amazing Charts will automatically count how many patients were returned in the search and compare with the total number of patients in that age range who were seen during the reporting period.
For more information on Search and Reports go to Help or use your F1 key.

Hi Guys.
Great responce Steven, that is it in a nutshell. The measure is tricky to read through. The easy version is you have to run a report, based on specific conditions, to generate a patent list to send reminders to. It is actually easier than it sounds. Here is an example of a report that you can do that will yeld a high percentage:
- Click the Reports button on the main screen. - Select the ?Meaningful Use Searches? option in the top left corner - Select the age range you see the most of (over 65 or under 5 years of age) - On the next screen, leave ?Any? highlighted, then click Ok - 2 teal criteria should be in the criteria field. You have to add one more - Select ?Demographics? from the Category section. - Select ?State? from the Field section. - Select ?=? as the Operator. - Enter the state you practice in under ?Value? (use the 2 letter code for the state) - Click Add Criteria to Query - Click Run Report - Once the report runs, you can print or export the report.
Amazing Charts will automatically count your numerators and denominators for you.
Please PM me with any quesitons.

This year I decided to use Menu 4 as one of the selections for Meaningful Use for our newest physician who began working in August 2012. Since forever it has been our practice to call every patient to remind them of upcoming appointments. When I tried to run a report on 1/16/13 for the last three months of 2012 I discovered that many of the patients did not have a selection for preferred method of contact. I have discovered that you cannot add telephone as a method of contact after the fact and have it counted by the Meaningful Wizard. Not only that, but the Wizard counted patients from all four of the physicians in our practice. When I contacted AC support I was told that I would have had to run the Wizard during the latter part of the 90 day attestation period for it to correctly provide the(Menu #4)numbers for a single physician.
