Simply put, Nuance doesn?t want you to use any version of NaturallySpeaking other than Dragon Medical in any popular EMR. With the release of Ver. 12, the list is growing. I feel your pain as I would also prefer the carrot approach over the stick approach by promoting other Dragon Medical features but this is Nuance?s marketing plan which we cannot change.
Dragon 12 may appear slower but it really depends on your computer. On many computers, Dragon 12 will actually perform faster. It?s approximately 20% faster on our i7 Lenovo with 16 Gb RAM and SpeechWare 3-in-1 microphone. While BestMatch V requires more horsepower, it also introduces true parallel processing. NaturallySpeaking 11 and the current Dragon Medical 11 feature BestMatch IV dual core processing but the cores have to take turns which means that only one core works at one time.
I?ve been testing Dragon Pro 12 BestMatch V for approximately 5 months (the 1st 4 months beta testing) and can tell you that BestMatch V still needs a little tweaking but if you are experiencing latency issues, try creating a new profile based on BestMatch IV which is also improved over Ver. 11.
The Dictation Box popping up (inconveniently) is a new feature (not a bug) that is designed for new users. However, experienced end-users will probably find this new feature to be annoying. I know like did. Fortunately, all of the new features include a toggle on/off option. In fact, even many of the previously introduced Ver. 11 features, (but not always desirable) can now be toggled off. This is also the 1st. If you are using KnowBrainer 2012, say ?Miscellaneous Options? to open the DNS options to the Miscellaneous tab or do it manually by clicking tools/options/miscellaneous. Now remove the checkmark from the last box labeled ?Use the Dictation Box for unsupported applications? to eliminate this feature.
Premium 11 has already been pulled from the market in most areas although you might still find it at Best Buy?s, OfficeMax and Staples. While Premium has the same speech engine as DNS Pro, it is not network savvy, isn?t designed for professional use and in all likelihood, every time you manually cut and paste from NotePad to Amazing Charts, your microphone will be turned off which means that after you click back into NotePad, you will have to manually turn the microphone back on. However, if you wish to stay with Premium 11/12 or even Pro 12, you can add optionally KnowBrainer Medical 2011 ~

Adding KnowBrainer to DNS Pro or Premium 12, in many ways, is equivalent to Dragon Medical 12 on steroids.
KnowBrainer Medical includes 11,000 commands including several commands that are designed to deal with EMRs which prevent the use of the Dictation Box and turn off the microphone. All you have to do is launch NotePad, by saying ?Start NotePad?, perform your dictation as you currently do in the Dictation Box and when you have finish editing, say ?Transfer? which will cut all of the text from NotePad, close NotePad, paste the text into AC, tab to the next field in AC, re-launch NotePad and turn the microphone on (because NaturallySpeaking automatically turns it off). Then perform your next dictation and say ?Transfer? to repeat the entire previous process. When you get the last field substitute ?Transfer Out? (to prevent NotePad from reopening). While we would prefer the ability to directly dictate into Amazing Charts, that ability cost $1600 and limits you to Dragon Medical 11. Our process allows you to use Ver. 12 but again, if given the choice, we would actually prefer to use Dragon 11 because no matter how slick we make our cut and paste commands, nothing beats the ability or user friendliness of dictating directly into your EMR.
Additional note: To further rub salt in the bone, only those using Dragon Medical can even dictate directly into this forum. I definitely feel your pain
