Go to the Medicare EHR Incentive website and sign in using your using your NPPES web user account. Once you login for the first time, you have to register, then go through each step, filling in the data from the AC report. If you have issues with your User ID/Password and are unable to log in, contact the EHR Incentive Program Information Center at 888-734-6433.

We have done that. Don't understand the attestation worksheet. We got an appt with Dina tomorrow. Hopefully we'll get it figured out. Thanks John
