Hi Karl,
Welcome to Amazing Charts, if somewhat belatedly. That would have been a pretty scary event. Here's my take on it. Just another one of my opinions.
It is still more likely this event was due to a rather weird combination of user entry than an inherent problem with AC. Backing your program and/or running AmazingUtilities would most likely have no bearing on it either way.
I wouldn't go through the whole process of calling Tech Support, et al, unless it were to happen again. A lot of users have used AC for a long time and thousands of scripts have been printed and, if this were a bug in AC, it seems as though it would have surfaced before. I think posting here and seeing if it has happened to anyone else WHICH YOU DID

is the best course of action.
There used to be a problem with AC ALONG with a user entry issue that caused two patients to populate the subject line of messages. It would actually enter one name and then two of the other. I can't even recall what series of events had to occur for it to happen but it happened at least once a week.
What probably happened is that there was some user entry issue ALONG with the fact that AC allowed those entries to produce this problem. I have tried to reproduce the problem, and as those who know me on the boards, I will probably continue to

Best of luck! And welcome again.
