so if the mu wizard shows that i have achieved all the goals does it mean i am ready to attest or is there something else to do??

Nope. Go for it. It is easy.
One thing happened to me, though, that you should look for. Make sure none of the numerators or denominators are 0. AC Wizard seems to pass you if that is the case while the MU program at CMS does not.

what about both numerator and denominator both being zero like pt asking for electronic med record??

I haven't attested yet, so I don't know from personal experience. But my REC guy said that situation is OK for the required Core measures, but you can't use one like that for one of the 5 Menu items that you choose.

Correct.
You can just choose the exemption for the core measures, as I did, but not for the menu items. It works fine.

Sharlene and Bill are both correct.
You could "pretend" that a patient asked for the record and click the MU buttons in AC to document this in order to make the numerator and denominator not equal "0".

Clem Surak: Is anyone else using ACPM and the EOBs are not matching up to patient accounts and the EOBs are having to be retyped into ACPM? Started a couple of weeks ago. Payer IDs are correct and match the EOB payer IDs.
tcosta: @ Clem Surak: yes, this has been going on for MONTHS in our office. We've been told it's the payers fault and it has to do with loop segments and payer response. Doesn't make sense to me, considering it's ALL the time now on multiple EOB's a week.
tcosta: It almost never used to happen and now it's constant.
tcosta: ACPM is extremely short staffed and it's taking them days to respond and fix the issue.
Clem Surak: Yes, it has been getting progressively worse. I have a ticket logged, but have not heard back, so I keep retyping the EOBs so that our accounting is up to date. Hope the migration to the new Clearinghouse fixes this issue. Thanks for the response.
