I wish when a record is pulled from a specific date on the schedule, the date and time stamp remains. Eg. Pt. is seen on a Friday. You don't record the visit. You come back on Sunday night and try to catch up. You pull up the chart from Friday's schedule. Now the date and time stamp is from Sunday Night. Of course, you could change that. But, I cannot recall how many times, in a hurry, I have forgotten to do that. So many of my patients were embarrassingly 'seen' on Sunday Night. The visit now is wrongly recorded, billing is erroneous, and now another "addendum" is to be added recording the error. Could there be a fix?

Zak, you are right on with this one. I have the same issue maybe from a slightly different perspective.

For instance, I can click on a patient's name and see that I saw him one month ago, say 11/14/07. But, if I open his chart, it will always have today's date in bold at the top. It is very confusing.

Yep. Vicky has a number of patients that seem to have come into our office with her at some godawful time on Sunday/Saturday morning. Even holidays--when she was in DC.

Luckily, the real date is at the bottom, but I do find it very confusing. I would like your take on the fact that it seems somewhat of a medicolegal issue as well where you can just make up a note and date it back.

Maybe that should only be addendums where it states addendum and makes it very clear this is an ADDENDUM to a past note.

Well, for those of us who have providers who catch up of charting at weird hours and time off, we have been bitching for a way to fix the date, kind of like a line thru it kind of thing, so as to not look like we are doctoring the record or anything for the longest time. I would even say that if the chart was saved the same day, there should be small window of opportunity to change the date or add a few lines after the fact. It seems like the moment one prints the invoice is the moment the you realize that you forgot to change the date.

We have been asking for something like this forever. Vinny, are you there, AC are you listening???

You are definitely onto something here. What's wrong with a "line through" that would automatically place a time stamp and initials on any change, just like we do with paper?

At one point this was something that even AC said they were looking into, folks wanted it, and then it just sort of went dark. Perhaps we should start a new "Line thru it" thread to get it back up on top again. In general we have wanted a way to allow a doc to say; "oh darn I forgot to add that one symptom or last instruction I told Mr Smith" and then allow them to add that extra line or two to that already saved encounter, not a separate addendum or visit, like we presently have to. As you said it is a paper thing that providers do every day, so it would only make working sense to get such a feature into any good EMR. And we still want to be able to just change the darn date of the encounter. How many times have you tightened up charts on weekend and forgot you were really finishing up that last Thursday's visits??? Happens all the time, but in paper you can quickly fix, but not here right now.

Date issue? Anyone have an issue with my dating? Does anyone have any idea the last time I dated?

I asked out this gorgeous, extremely intelligent, very rich girl the other day. She picked me up in her Lexus and we went out and had a nice dinner. I asked her if we could stop by the office so I could pick up some meds from the sample closet. I still don't know why she got so mad when I came on here for a couple of hours. Don't people know what a good time is. She kept talking about going back to her place. Why would I want to go there rather than staying here at AC-ville? Of course, maybe she had Internet; now that would have been perfect.

My daughter is in the "Theater" tonight! She is a SnowFlake in the Moscow Ballet's traveling NutCracker. They train and use local kids as they travel across the country and then only use traveling regulars for the main characters. My question is, what samples could a Pediatrician out on a date need or even have available from his sample closet? A Urologist, maybe. A Pediatrician, God help us.

You really need to get out more.
Concerning the line through it issue; I sure could have used it a couple of months ago as I began using AC along with Dragon Naturally Speaking. Following a busy week of dictating and signing, it was a rude awakening to realize that the only edit I could do was an addendum. Currently, I forward the charts to myself after I dictate and proof read in a quieter moment.
The line through a mistake makes great sense.

I love Dragon Naturally Speaking with Amazing Charts. They go together like peanut butter and chocolate! However, you do have to check things over.

I console myself with the fact that typists have their typos, and even the professional transcriptionists I have used have produced some bizarre transliterations of my spoken words.

I find my accuracy went way up when I sprang for DNS 9.5 Medical version, and a better mic.

However, I have left behind some funny errors, even at that.

Okay, I don't have a fix, but I *do* have the workaround, thanks to the unorthodox way I am using Amazing Charts right now.

Once you have popped open that chart from the schedule, it is stamped with the date and time it was opened, no matter what you do with it from that point out. All you have to do is open it on the date of service, and forward it to yourself to dictate later.

When my patients arrive, my receptionist forwards their chart back to our nurses station. The nurse calls the patient, opens their chart with her tablet PC at our vitals station, and records their vitals and chief complaint, at the very least. Then, she forwards the chart to my inbox.

As long as the chart is opened and handled once on the date of service, it is stamped with the proper date. When you pop it open on Sunday night, open the forwarded chart from your inbox instead of the schedule, and it will have the correct date and time.

Brian is correct, that is the way to handle the problem (when you sign the note it will ask you if the DOS is correct) unfortunately, if you are like me, I print out a letter to the referring doc for 99% of visits and that date is automatically dated on the day you are typing it which can make it look like you are seeing people on the oddest dates---then again there is a work around by adding the correct date of service in the body of the letter. Nothing is perfect is it?
