Has anyone ever seen or suspected that the yellow note on top of a chart was for the wrong patient? That is you are looking at patient chart A and it contains a yellow note for patient B? We have 2 providers within a few days who have seen charts where they swear the yellow note was from some other patient.

I told them it was exceedingly unlikely bug. And if there was a bug like this it would have been noticed long ago. Nonetheless I have this complaint. Is there maybe some usability issue that would lead a provider to mistakenly enter the yellow note into the wrong chart? Aside from just a sleep deprived provider?

Maybe they selected the wrong patient? that's a more likely scenario

Sleep-deprived or not, the provider would have to enter the yellow note in the wrong chart.

Just a thought but if you make a reminder, you can choose to have it populate the yellow sticky. Of course, it will go to the patient you chose, but maybe one wouldn't be paying as much attention while doing that.
