We ran into a problem--we had one provider until the past 3 weeks when we added a new provider. When we went to print out the CMS-1500 forms for the new provider, it printed the other provider's name under the "signature" and name of physician even though the new provider signed the chart note and billed it. Has anyone noted this problem? How did you resolve it? Any advice will be appreciated.
