Has anyone successfully imported a schedule from Sage Medical Manager into AC?

One option which I have been playing around with but have not been able to make work is to use the Appointment Quest import option. I can create a CSV file with all of the appointment information, but do not know how appointment quest formats their CSV file. Does anyone have an example AppointmentQuest CSV file?

does anyone have any other options? (besides manually entering a few hundred or more appointments)
