UPDATE ON PRIZE! Due to ever tightening land use resitriction those who want NYS roughest terrain will meet me on Kingsbridge Road at the interestion of Kingbridge and Jerome Avenue in the Bronx for a tour of the South Bronx. Means of protection not provide, all fire arms must be licensed for use and carry in the state of NY. A Zero Tolerance policy will be in effect for all non-perscribed controlled substances. Those who would choose the Rural Ride, will meet me on a Saturday Morning yet to be determined in Joliett, Tower City, PA at the enterance to Rausch Creek Off-Road Park for a day of Fun and Mud. Fire Arms not allowed and hunting banned on premises. All trailrides to be restricted from dawn to dusk. NO Nighttime Wheeling allowed. All passengers must join and become member of Rausch Creek and sign both the Rausch Creek and the Paul Blei Off-Road Tours waivers of Liability. For either Tours any additional passengers will be charged at prevailing rates, limit of two additional passengers slim enough to fit on either side of my son Adam's Britax Marathon carseat (gotta bring the kid, right? "Daddy we Shot the Hole!!!") There will be no alcohol allowed on the trails or in the Jeep at anytime and is a violation of NYS law and Rausch Creek policies. Roy, Seeing that I don't "go that way" ("not there's anything wrong with that.." J.S.) just how do you know what my "terrain" looks like??? Drinking and Wheeling is no better than drinking and regular driving or drinking and boating my friend. That's how trails get destroyed and therefore closed, and folks get hurt. By drunk disrespectful yahoos. "Tread Lightly" But kicking back with a few back at camp at the end of the day is always cool. But not on the trail. Hey, don't forget who is putting up a Jeep and all that gas. As we say at the trailhead; "Lock and Roll". Roy, No need to appologize, I was just busting your chops in return for a good laugh. Actually, I got a real kick out your joke and "my terrain". Just thought I should "ride" the joke for a bit longer.
Is my terrain on a global positioning system too, so I can no longer go underground and off the grid???
And yes there are tread lightly off roaders who want to help maintain and preserve our natural resources and not just chew them up. Many of us join in on trail repair days and give back to the land days. I just think that this land was really made for you and me (ah Woodie) and that somehow we should be able to share it and find a way to make most of accessable to folks all while preserving it for future generations. Doesn't a wheeler in a wheelchair have as much of a right to go see ghost towns and mountain streams as you or I? Little kids like mine who can't hike in 30-40 miles with a heavy pack on their back?
By the way, I guess nobody got the joke: "...Not that there's anything wrong with that." It's a quote from a Seinfeld episode where gay and alternative lifestyles kept coming up and they kept ending half their commments about such things with "...not that there's anything wrong with that". As I have expressed to Bert, I think IMH is a program with great promise that fails in application.
I have been distressed with the location it chooses to insert some data, and so far I don't know how to fix it. Too much information goes into the “chief complaint” line for easy review, for instance. I would like to be able to customize where information gets inserted into AC.
There is also too much “verticality” to the data fields: for instance, you might get a line that says “Duration:” and then a line break and a new line that says “two weeks”, and then a double return, and then another line of text. This adds up. Very quickly, your data is extending off the visible field of the HPI. That's fine for the printed version, but it is inconvenient for me to review in the data entry screen. I would like to see the format of inserted data change to conserve space in AC.
Also, I don't care for all the templates, and I have not figured out how to edit templates yet. Truthfully, I haven't had hours to spend figuring it out, but the fact that I need hours to figure it out says something.

You never cease to amaze me with your writing ability and conciseness. You should definitely be the president of a group of physicians and providers dedicated to .... well I don't want to be acused of antitrust.
When I worked with IMH three years ago, I must have sent thirty emails back and forth to Matthew Ferrante. He's a nice guy, but I don't think he has a tremendous amount of input to the design. Once again, it's probably a software program that is designed more by programmers and engineers and not physicians. If Jonathan, Vinny and Mr. Ed worked on it, it would probably make sense.
I was using an online survey program once for making surveys for the hospital, and I even used HotDocs, a program for making active documents generally used by lawyers (yuk!, sorry). But, I figured it out rather easily. I would rather put more work into it myself and be able to customize it, then the other way around. It's too bad, because it could be a killer program.
The more important question is why are we debating this at 9:30 PM on a Friday. Wouldn't it be cool if we and a few other posters could all sit around with some steak, lobster and Heinekens or Buds if that's your beer and talk football and the ACC instead of AC?

Aarrrgghh... You guys are killing me with this good food and beer talk. I am on-call and behind on paperwork. I just barely had time to meet the family for buffet egg rolls and Szechuan Beef. Coastal cuisine in my neck of the woods means heading south for Gumbo and Jambalaya. I've never had fresh lobster. Now I'll admit that Heineken is pretty tasty, but a good friend from residency introduced me to Spaten, and after a long separation, I found it in a Wal-Mart SuperCenter -- in southwest Missouri, of all places -- just last week. I consider it to be one of the finest lagers in the world. Completely unlike its big brother, the bloated Logician, which turns a conjunctivitis into a five page monstrosity. yep, was a FANTASTIC program.. but they got greedy, sold to Medscape, Medscape couldn't figure out how to make money with it, gave it back to the original people, who sold it to GE, who promptly turned it off. Why would anyone in their right mind pay them $50,000 when their $100/month/doc package did a much better job? They STILL haven't fixed their product, but they've done a good job of eliminating the competition by buying them out. THIS is why I will never again use any application critical to my business that isn't locally hosted. I miss Logician Internet... V.

@Vinny Could not agree with you more. I think Logician Internet was the best. Could have been great. It was a drill down but fast and could code for you. Plus I had others, and we could share information. A lot of people lost a lot of information on LI.
Somehow I was able to download their program and use it locally. It worked pretty well. But, with Logician, I still to this day do not know how they get people to buy it. I hate it when someone transfers to me from an office who uses Logician. Get ready for a 500 page record. Luckily we are digital. I have found that their notes are more about repeating every part of the note in total and complete detail such as SH and PMH. I think they only worry about documentation almost like they are writing every note in anticipation of the lawsuit of the century.
@Brian I have no arrangement. Just cover my patients all the time. I do have a friend in FP who will cover me and my patients if I am sick or just want to sleep in. Haven't taken him up on the latter yet. But, I use him for the insurance papers. But, if you want to move to central Maine, we can take call and you can have fresh Lobster.

However, when I saw "Portland", I though maybe you guys were going to visit the Pacific Northwest. There's more than a few of us out here in WA State alone.

My vote is for the area close to the Grand Tetons. I put Paul in charge of setting up the accommodations, i.e. tents or hotels. That is one of the most beautiful places on earths.

Guys, Now this is a practice manager's question. My dear wife who in principle I agree with although I'm scared CHITTless that we will starve to death, wants to try and go "No Insurance" what so ever. So two questions come to mind. 1) If one does not take insurance does one still need to have call coverage by most laws? Is this also a hospital privilages kind of requirement too? If we don't admit our own, but turf to hospitalists anyway, do we even need privilages anyway? 2)For docs to really be coverage for one another, do those docs have to have the same or extremely similar panels of carriers that they PAR with, so as to be able to actually see eachothers patients in a real pinch. Especially with managed care in primary care, because in general these folks are supposed to only see PCP's who are their declared PCP, right? Can one doc cover for and see the other guys managed care patients if he is the coverage of record even if he doesn't PAR with that plan??? And as a side note. How come with "exclusive" in network provider type contracts these are not seen and ruled as "restrictions of freetrade". Both patients and providers want to be able to do business out of network. Even if a patient wanted to see us out of network and was willing to pay the entire freight themselves, they would still be SOL, because if and when they need referal, prior auth's, and fight for drugs on and off of formulary, these too can only be done by a PCP that is in network. It would seem to me to be an obvious restraint on freetrade, but then again I'm not some lapdog appointee, on the FTC, who is beholdant to politians who are beholdant to large insurance carriers, so what do I know, right??? But it seems obvious to me that if the primary care specialty organizations had any balls (  ) at all, that they would combine forces in a joint, class action against such as a restraint on freetrade, refuse to settle nicely even if it takes a generation to see it through to it's end, and take it all the way to the federal supreme court to finally get these exclusive contracts deemed as such. The class would be all PCP's who are out of network and all potential patients in all national and regional type plans. That is a huge class there. How many Americans are in or have been in some sort of a managed care plan? But on a happier note...Bert you are a tease...There are few places left in America that I want to revisit more than The Tetons and the surrounding country. I haven't been there since I was trout fishing and going cross country with my folks in the backseat of a fairly new, 225 slant-6, 4 door sedan, Dodge Dart. Oh the cutthroat were so beautiful and tasty. Bonus points to any member of the board who is not from out there or has visited recently (Leslie I'm sorry but that means you too...) who knows why they are called the Grand Tetons and what does the name mean.... Ironic you should mention that. My MA told me the origin the other day. It has to do with the French explorers from Canada who named it due to its looks. They called it Grand T... but this is a family board so I will leave it at that.

Geeze...I have been there several times and never even considered the origin of the name. I even named a mustang stallion "Teton" because I love the area so much. I also have a Donkey named "Denali" which, I do know means "High One". Do I get 1/2 point??
