In the past couple of months, I have had entries that I know I have made in a chart after a visit disappear. My CMA will go into the program after I have signed off on a chart and there is nothing there from the newest entry...no vital signs, no entries by me....nothing. I don't know where it went to but it makes me worried. Any ideas?

And it's not from 10 pm to Midnight??? lol FYI, In case you missed it our messeges were disapearing but only for two hours a few versions ago. Just thought I'd help you have a laugh.

I guess I am not sure. We used to have a problem where the four digit number of the patient would somehow revert back to a three-digit number. So, say the patient was 1059, if it became 999, then it would be in the database but hidden. I think this was fixed, but who knows.

If it's happening a lot, you may want to temporarily forward it to yourself so at least you have it in case it goes away.

This sound very concerning. Lost charts or encounters are no laughing matter as much as I did try to put a smile on your face. Have you called tech suport yet? This sounds like a job for the mothership to me. If they haven't already, they can securely upload your database and look it over for issues. How that will proceed is anyone's guess but it is certainly worth a try. Please let us know how things go and what you find out. Best of luck on all this....
