How do you folks enter hospital encounters, especially for patients who stay for several days?

The bulk of the notes are on the hospital's system, of course. So you really don't want to re-enter them all into AC. It's also useless to do so.

So, how much info do you enter into AC, and then how do you handle the subsequent encounters?

Do you create a claim for each visit, or do you just make notes on the face sheet (carry it around) and then enter the charges at the end? Any other ways?

all days are entered as separate encounters in our office.
