We are often asked to fill out a vaccination history form for some exam clearance I have to sign, usually for school or camp or sports. I usually write "please see attached" and print out the kid's vaccination history from AC.

In V4, this printed out in a vertical list with my practice letterhead at the top. Now with V5, it is printed in a different format, which is much more like the format of the forms I'm asked to fill out. Great! But no practice overhead--so I've now had a couple of parents tell me that the school is asking where this printout is from, perhaps a little suspicious that the parent has made it up.

It would be nice to have my letterhead appear at the top of the page.

Yeah, I know an industrious parent could fake that, too. But this doesn't seem unreasonable. Thanks.

Yeah, in V5 its a little excel spreadsheet in 6pt type. I much preferred it the old way with the practice header and 12 pt type (or was it Courier font).
